Registration Documents

In order to register your child for little league, you will need to provide the following documentation:

Required for Registration:

1. Actual Birth Certificate (not a copy)
2. Any TWO forms of identification that prove Residency in Tempe South Boundary
   • Utility Bill (Gas or Water but not both)
   • Cable Bill
   • Driver’s License
   • Something from a company delivered through the mail (credit card, insurance, phone)
   • Motor Vehicle Registration
   • Voter Registration Card
3. Drivers License (for volunteers)
